• The provision of a hot daily delivered meal service which incorporates a safe and well-being check on the person and their living environment.
• Home from hospital assistance, supporting older people on discharge from hospital.
• Working in collaboration with partners to help avoid hospital admissions/re-admissions and to provide support to enable a return to independent living.
