In the winter of 2021/22, We started a project called H2H which is Hospital to Home, which is a 7 days funded care package of hot main lunch time meal, dessert, and a tea pack for the evening provision, including the wellbeing check of course which is free of charge to the Service User.  This has been massively funded by the Suffolk Community Foundation and their doners.  This has been and is a vital project that enables the NHS discharge teams to free up beds, elevating some pressure on the NHS but also supporting the elderly and vulnerable to get back into independent living and the community. 

The project has been extremely effective, we have not only been able to support over 400 elderly and vulnerable but that's also the number of beds we have been able to free up for further patients.  We are extremely blessed and grateful for their continued support THANK YOU
